The large integrated healthcare network successfully concluded its second annual charitable giving program across multiple states 

SEPTEMBER 10, 2024FORT LEE, NJ – CareOne, a leading, family-owned senior care provider with more than 60 locations in the U.S, ended its second annual Summer of Sharing charitable giving program, raising funds for local charities. 

CareOne created Summer of Sharing in 2023 in an effort to affect change on a local level and mobilize its facilities across multiple states to give back to the communities it serves. Last year, the healthcare network raised $60,000. 

Earlier this summer, each building selected a local cause or charity for which to raise money. Over the past three months, buildings have organized fundraisers and engaged with their community networks to support their causes. 

“Summer of Sharing is our yearly opportunity to invest back into the communities that have always embraced us,” said CareOne Chairman and CEO, Daniel E. Straus. “I am proud to see our teams mobilize and extend our commitment to care beyond their four walls and make a difference in the lives of those who may need a little extra support.” 

CareOne selected more than 30 local organizations that support a variety of causes, from food banks, family services, housing and beyond. Fundraising efforts included facility events such as family fun days, “paint and sip” parties as well as car washes. To close out the initiative, many CareOne locations hosted celebrations honoring their beneficiaries to present them with the funds raised. 

“Giving back to the community is a core pillar of the CareOne Difference,” said Anne Marie Gauntlet, SVP of Operations for CareOne. “We are thankful to our employees, residents and patients who dedicated their summer to their causes. It was heartwarming to see the CareOne community work together to further support the communities they serve.”

This year’s goal of $75,000 is aimed at extending that impact to create lasting and positive change.”

CareOne is pleased to share that CareOne at Newton’s Director of Nursing, Rachel Yerokun, RN, has been named to the 2023 Frontline Honors Class by the Aging Media Network in the Skilled Nursing (SNF) category.  

The Frontline Honors Awards celebrates the exemplary character and performance of frontline workers across the care continuum. Rachel’s recognition is a testament to her commitment to providing the highest quality and most compassionate post-acute care. 

About Rachel Yerokun

Rachel’s skilled nursing career journey began in 1994 as a CNA and quickly accelerated when she took advantage of tuition reimbursement to obtain her nursing degree. In 2003, she was hired as a unit manager for a subacute facility with 44 beds. Under her leadership, she transformed the facility into a model of excellence, recognized by state inspectors for its complete deficiency-free status. 

Additionally, Rachel was instrumental to CareOne during the COVID-19 pandemic. She oversaw one of the first COVID-dedicated units in Massachusetts and the first of its kind for CareOne. Under her direction, Rachel’s facility reopened two units to treat ill patients. She never turned anyone away. Rachel frequently worked around the clock to provide necessary care, often admitting 12-13 COVID patients each day. Rachel was the first to cover for her fellow nurses and CNAs when they were sick, and she helped her staff through clinically and emotionally challenging situations. Her agility was crucial to the health and well-being of her patients. 

Rachel’s colleagues often describe her as compassionate, hardworking, inspiring and selfless. Her infectious smile radiates positivity and warmth to all those she encounters, which is an essential quality to possess as a healthcare professional. Rachel’s work ethic is a driving force behind the success of CareOne at Newton. We are very fortunate and grateful to have her.

CareOne is delighted to share that Senior Dementia Care Specialist, Joan DiPaola, has been selected by the 2024 McKnight's Pinnacle Awards as their Thought Leader award recipient.

McKnight’s Pinnacle Awards honors industry veterans who have set standards, driven change, and provided guidance and inspiration to peers. This program recognizes individuals across the senior care, skilled nursing, or home care sector at large.

With more than 20 years of experience in the fields of nursing and dementia care, Joan has earned a reputation as a champion of person-centered care. Since joining CareOne, she has taught frontline staff, physicians, and communities how to improve communication with those with dementia to foster connection, break down social barriers, and remove stigmas. She developed CareOne’s Food for Thought program, an educational initiative that teaches the language and the approaches to effectively communicate with those with dementia without pharmaceutical interventions.

Additionally, Joan organizes monthly virtual networking sessions for caregivers who have loved ones with dementia, enabling families to directly confide in Joan about their challenges while also connecting with other caregivers in similar situations. As a result of these networking sessions, many caregivers have formed long-lasting and therapeutic bonds with others navigating the same challenges.

Another initiative Joan developed is a monthly Spousal Elegant Dining program, enabling spouses and their partners with dementia to dine in a restaurant with other couples experiencing the same challenges. This includes working with dining staff in local restaurants and educating them on dementia – including best practices for communication as well as nuanced considerations like choosing the right table settings to facilitate the most comfortable and inclusive dining experience.

As an extension of this effort to give loved ones with dementia more freedom of choice and personal autonomy, Joan also introduced summer music concerts and other activities, empowering residents to choose how they spend their time.

Joan has also been integral to refining CareOne’s approach to care. She introduced monthly educational training sessions and continues to counsel staff on how to develop personalized care plans for every single patient at CareOne, including comprehensive hand-off notes, insights into patients’ life stories, and other important personal considerations.

We are grateful to Joan for continuing to raise the bar for dementia care. Congratulations, Joan, for receiving this exceptional recognition from McKnight's Pinnacle Awards!

One of our amazing employees is Tyra Fields-Gary, the Senior Director of Nursing Services at CareOne at East Brunswick, who has worked at CareOne for almost nine years. We wanted to take a moment to highlight her impressive career and her invaluable contributions to our organization, her fellow colleagues, and her patients and their families.  

Tyra’s “Call to Care” 

As a child, Tyra and her sister would visit the local nursing home with their mother and provide church services and seasonal gift baskets to the seniors in their community. This ritual ignited Tyra’s passion for nursing, one that she knew would become her professional calling. Tyra received her diploma in nursing at the Winifred B. Baldwin School of Nursing in Essex County, NJ. She then obtained her Bachelor of Science in Nursing (BSN, Clinical Management) and her Master of Science in Nursing (MSN, Transcultural Nursing and Nursing Administration) at Kean University in Union.

After completing her degrees, Tyra began working in acute care at a local community hospital. There, she cared for patients who had just left the ICU, first as a staff nurse and then as a charge nurse. During that time, she took a per diem position as a charge nurse in a long-term acute care hospital (LTACH), where she worked with patients with high acuity and complex care needs.

In this role, Tyra quickly rose through the ranks from clinical manager to Director of Nursing, eventually making her way to Senior Chief Clinical Officer. While working in the LTACH, she discovered the post-acute world, developing a strong professional relationship with CareOne at Wayne, the local post-acute CareOne facility.  

Tyra’s Journey to CareOne 

After 18 years of working in an LTACH, Tyra was ready for a change. She yearned for a role that allowed her to see her patients’ care through to the end. “Seeing a patient go home after an intensive treatment plan was so gratifying,” she said – and she wanted more of it.

What appealed to her most about CareOne and the post-acute world was that she could play an instrumental role in seeing her patients’ care through. “Very often in acute care, you’re wondering: How did that patient fare? Did they do well? Did we meet the needs of the patient and their family?”

At CareOne, she loves that she can see the residents through the care continuum. “I can still use my clinical skillset and feel productive as a clinician,” she said. Being able to bridge the continuity of care for her patients who come from an acute environment and understand the unique physical needs of LTACH patients offered Tyra the perfect balance of her seasoned LTACH experience and a new professional challenge. 

When Tyra was applying for a role at CareOne, she was met with open arms. “I met with the building administrator, VP of Marketing, the CCO, and even the owner of CareOne – all even before I accepted the job. I thought, ‘This doesn’t happen in the acute care world!’ It was so important to me. Everything felt right.”

What instantly struck Tyra about CareOne was that it felt like a “very warm, very kind, and very open” place where staff not only looked out for their patients, with the primary focus being on quality of care, but also looked out for each other. 

Long-lasting Bonds Built at CareOne 

Tyra’s favorite part about working at CareOne is her residents. “I love the relationship I have with my long-term residents and the impact I have on my sub-acute residents.” For her sub-acute patients, seeing those who had been bedbound when they were admitted to CareOne walk out the door on their own to go home is “so fulfilling.”

Of her long-term residents, she says, “[They’re] part of my family. They ask about my family, how my kids are doing. And my kids ask about my residents.”  

While Tyra builds meaningful relationships with all her residents, there’s one that is particularly special. “I’m very close to our resident council president. About four years ago, she renewed her vows and asked me to stand in as the mother of the bride because she felt like I assumed that role [for her].” To this day, they remain extremely close and can often be seen around CareOne at East Brunswick together, with the resident calling Tyra by the nickname she gave her, ‘Mama T’. 

Her Personal Connection to CareOne 

The comfort CareOne has provided to Tyra extends beyond the meaningful relationships she has built with patients and coworkers during her time there. It has also had a profound impact on her family. CareOne at East Brunswick holds a special and deeply personal place in Tyra's heart because it was her mother's home for three years before her death in 2022. It was a difficult decision to bring her to CareOne, but Tyra knew it was the right one.

“I felt 100% comfortable bringing her to my center. This building and staff became an extension of my family because they cared for her like they would their own mothers.” This gratitude is part of what drives Tyra every day. To improve quality of life and make a difference in a resident’s and their family’s lives, it is her mission to ensure every resident receives the same care she knows her mother received.

“I love being a nurse. It reminds me even on my worst days why I still love it.” 

A Place to Stay and Grow 

For Tyra, the opportunities to grow professionally at CareOne are endless, saying, “[CareOne] goes above and beyond.” With encouragement from her leaders and financial support from CareOne, she received her Assisted Living Administrator certification (CALA) and knows that if she ever wants to step away from her current clinical role, she has the support, ability, and certification to do so.

Tyra has also mentored new DONs at other facilities, training dozens in her nine-year tenure (and counting) at CareOne. She loves being a mentor and takes great pride in seeing colleagues under her leadership get promoted: “It makes me feel good!”
