Liz Patterson-Schum, ACBSW, C-SWCM is a certified social worker with a Bachelor of Science degree in Social Work from SUNY Buffalo. She is a Certified Geriatric Care Manager/ Aging Life Care Professional™ and a Certified Dementia Practitioner.
Liz has been in medical, geriatric social work for more than 35 years. She has devoted her career to working with older adults and their family. Her specialized training, certification, and professional work experience has provided the knowledge and problem solving skills needed to effectively work with those in need. Liz has expertise in the aging process, community resources, our healthcare system and the unique challenges that caregivers face. She is committed to preserving the rights and dignity of her clients.
Liz has worked in hospital and nursing home settings. She has homecare experience from a certified homecare agency, and a physician based medical management organization where she was part of a specialty geriatric team and managed a "Geriatric Clinic Assessment Program". Liz was also a Geriatric Care Manager in an Elder Law attorney's office, and was the program director for a community based geriatric care program.
In 2011 Liz was awarded the "Career Achievement Award" by the Rochester Business Journal, for her distinguished work and service.
